2 passport sized photographs of the minor to be endorsed on the reverse side by the consenting parent(s).
Copy of online acknowledgement slip and payment slip.
Evidence of Nigerian citizenship of parent(s) or grandparent(s).
Birth certificate (certificate must be issued by the National Population Commission for minors born in Nigeria). For person(s) NOT born in Nigeria, a copy of the birth certificate/population extract issued by the country of birth.
Letter of consent from either parent(s) or guardian(s).
Copy of payment of administrative fee paid to Embassy’s account.
Copy of appointment slip.
