Copy of completed guarantors’ form, and attached passport photograph of the guarantor.
Copy of data page of guarantor’s valid Nigerian passport.
For a person NOT born in Nigeria, a copy of the birth certificate/population extract issued by the country of birth. (Original may be requested for sighting)
Letter of identification from Local Government/State of origin or National Identity Card.
Marriage certificate, where applicable.
Copy of payment of administrative fee paid to Embassy’s account.
Copy of appointment slip.
