Reissue of damaged passport

1. Application letter addressed to His Excellency. (see sample)
2. In case of fire accident, where the damaged booklet cannot to be presented, a copy of report issued by the police or appropriate authority is required.
3. Copy of completed online passport application form.
4. Copy of online acknowledgement slip and payment slip.
5. Damaged passport booklet to be renewed.
6. Photocopy of data page of damaged passport.
7. Copy of residence card.
8. Copy of payment of administrative fee paid to Embassy’s account.
9. Copy of appointment slip.

Reissue of lost passport

1. Application letter addressed to His Excellency. (see sample)
2. Copy of extract or document issued by police or appropriate authority for lost passport.
3. Copy of completed online passport application form.
4. Copy of online acknowledgement slip and payment slip.
5. Photocopy of data page of lost passport.
6. Copy of residence card.
7. Copy of payment of administrative fee paid to Embassy’s account.
8. Copy of appointment slip.

NB: Where a passport was declared lost and reissued, if recovered, the holder shall immediately notify the Embassy of recovery and submit the passport for cancellation.
