Change of name due to marriage.

1. Application letter addressed to His Excellency, (see sample)
2. Copy of completed online passport application form
3. Copy of online acknowledgement slip and payment slip.
4. Passport booklet to be renewed.
5. Photocopy of data page of passport.
6. Copy of residence card.
7. Copy of marriage certificate or declaration of marriage.
8. Sworn affidavit from a Magistrate or High Court of change of name deposed to by applicant.
9. Copy of newspaper publication of change of name. (21 days after publication)
10. Statutory evidence/document for change of name from issuing authority in the country of residence. (in lieu of viii and ix)
11. Copy of payment of administrative fee paid to Embassy’s account.

Change of name due to divorced

1. Application letter addressed to His Excellency.  (See sample)
2. Copy of completed online passport application form.
3. Copy of online acknowledgement slip and payment slip.
4. Passport booklet to be reissued.
5. Photocopy of data page of passport.
6. Decree Nisi and Decree Absolute or declaration of divorce.
7. In case of Re-marriage, a Certificate of Marriage with the Current Spouse.
8. Sworn Affidavit from a Magistrate or High Court of Change of Name deposed to by Applicant.
9. Copy of Newspaper Publication of Change of Name (21 days after publication).
10. Statutory evidence of change of name from issuing authority in the country of residence. (in lieu of  viii and ix)
11. Copy of residence card.
12. Evidence of maiden name.
13. Copy of payment of administrative fee paid to Embassy’s account.
14. Copy of appointment slip.

Change of name due to widowed

1. Application letter addressed to His Excellency. (See sample)
2. Copy of completed online passport application form.
3. Copy of online acknowledgement slip and payment slip.
4. Passport booklet to be reissued.
5. Photocopy of data page of passport.
6. Certificate of death of spouse from issuing authority in country of residence.
7. In case of re-marriage, a certificate of marriage with the current spouse.
8. Sworn affidavit from a Magistrate or High Court of change of name deposed to by applicant.
9. Copy of Newspaper Publication of change of name. (21 days after publication)
10. Statutory evidence of change of name from issuing authority in the country of residence. (in lieu of  viii and ix)
11. Evidence of maiden name.
12. Copy of payment of administrative fee paid to Embassy’s account.

Change due to Correction of Error(s), Replacement of name(s), Addition of name(s), Deletion of name(s), Re-arrangement of name(s)

1. Application letter addressed to The Comptroller General, Nigeria Immigration Service through The Embassy. (see sample)
2. If more than one [1] alphabet correction, attach evidence of online payment of a non-refundable administrative charge of N30,000.00 for change of data.
3. Copy of completed online passport application form. (Data entered in the application form MUST reflect the change or correction requested on the passport)
4. Copy of online Acknowledgement Slip and Payment Slip.
5. Passport booklet to be reissued.
6. Photocopy of data page of passport.
7. Copy of residence card.
8. Previous documentary evidence(s) to support the request.
9. Sworn Affidavit from a Magistrate or High Court, deposed to by Applicant.
10. Newspaper publication from Nigeria.
11. Copy of payment of administrative fee paid to Embassy’s account.
12. Copy of appointment slip.
